Advance your clinical expertise as a Nurse Practitioner conducting home health risk assessments on a per diem basis within Greene County, IN. Assessments typically take 45 minutes and may include annual wellness checks or post-discharge visits. Shifts can be scheduled in Stockton Township and surrounding areas, such as Linton, Bloomfield, and Jasonville, based on your availability.
Schedule:

- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate medical histories, current health conditions, and medication records
- Deliver patient education on existing conditions and prescribed medications
- Identify gaps in care and recommend actions to address them
- Documentation onlyno creation of treatment plans or prescribing of medications
Qualifications:
- Nurse Practitioner license valid in IN
- MSN degree from an accredited nursing program
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification required
- At least 6 months of experience practicing as a Nurse Practitioner preferred
