Job Description

Deliver high-quality, patient-centered care while enjoying true flexibility as a Nurse Practitioner conducting home health risk assessments throughout Alamosa County, including the communities of Alamosa, Blanca, and Hooper. This per diem opportunity allows you to take control of your schedule, making it ideal for clinicians seeking a balanced lifestyle and diverse clinical experiences.

Schedule:

  • Flexible shifts available seven days a week
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment to fit your availability

Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members during 45-minute visits
  • Review and evaluate patients medical history, current health status, and medication usage
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and offer professional recommendations to enhance patient health outcomes
  • No prescribing of medications or creation of treatment plans required

Qualifications:

  • Active, unrestricted Nurse Practitioner license in Colorado
  • Board Certification as an NP (AANP or ANCC)
  • Current Basic Life Support (BLS/CPR) certification
  • Master of Science in Nursing (MSN) from an accredited nursing program
  • At least six months of current NP experience is preferred
