This per diem role offers flexibility and autonomy, allowing Nurse Practitioners to design their schedules while providing essential home health risk assessments for Medicare Advantage plan members across Columbus County, NC, including communities such as Whiteville, Tabor City, Chadborn, and Lake Waccamaw.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current health conditions, and medication regimens
- Deliver patient education tailored to medical conditions and medication use
- Identify and document any gaps in care, providing actionable recommendations for follow-up
- Visits vary in nature and may include both annual health assessments and post-discharge evaluations
- No prescribing or treatment plan development required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
