Advance your career as a Nurse Practitioner specializing in home health by delivering critical care to Medicare Advantage members within Kay County, OK. This per diem opportunity allows you the autonomy to create your own schedule, offering flexibility to fit your life, with minimum weekly or bi-weekly hours based on your availability. Shifts are available throughout the week, Monday through Sunday, in Blackwell, Ponca City, and additional nearby areas such as Newkirk and Tonkawa.
Responsibilities:
- Conduct comprehensive home health risk assessments (approximately 45 minutes each) for Medicare Advantage plan members, which may include annual health evaluations and post-discharge visits.
- Review and evaluate patients medical histories, ongoing health conditions, and medication use.
- Educate patients about their medical diagnoses and prescribed medications, helping to empower their health decisions.
- Identify gaps in care, making appropriate recommendations to support optimal outcomes.
- Focus on assessment and education; no requirement to prescribe medications or create treatment plans.
Qualifications:
- Active Nurse Practitioner license in Oklahoma.
- Current BLS (CPR) certification.
- Board Certified through AANP or ANCC.
- MSN from an accredited nursing school.
- At least 6 months of recent NP experience preferred.
