Advance your healthcare career as a Nurse Practitioner delivering home health risk assessments to Medicare Advantage members across Clark County, AR, including towns such as Arkadelphia, Gurdon, and Amity. This per diem role is designed for flexibility, allowing you to create your own schedule and make a meaningful impact on patient care in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive annual risk assessments for Medicare Advantage plan members during home visits (approximately 45 minutes each)
- Review medical history, current conditions, and medications
- Provide patient education regarding medical conditions and medication regimens
- Identify gaps in care and offer recommendations to ensure healthcare needs are addressed
- No treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board certification through AANP or ANCC
- BLS (CPR) certification
- MSN from an accredited nursing program
- At least 6 months of recent nurse practitioner experience preferred
