Job Description

Deliver vital healthcare to patients in their own homes by performing comprehensive risk assessments in a flexible, per diem role. This opportunity involves traveling within Broome County, including Binghamton, Vestal, and Johnson City, to complete Medicare Advantage member visits. Each home health risk assessment typically takes about 45 minutes and may involve annual health evaluations or post-discharge follow-ups, offering meaningful patient interactions that support quality outcomeswithout the demands of ongoing treatment planning or medication prescribing.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling: Create your own shift pattern
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Carry out annual home health risk assessments for Medicare Advantage members
  • Review patients prior and current medical history, conditions, and medications
  • Educate patients about their health conditions and prescribed medications
  • Identify gaps in care and make recommendations to close them
  • Focus exclusively on risk assessment; no treatment plans or medication prescribing involved

Qualifications:

  • Active Nurse Practitioner license in the state of New York
  • Current CPR (BLS) certification required
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months of NP experience preferred
