Step into a dynamic healthcare environment as a Nurse Practitioner specializing in home health risk assessments. This per diem role offers flexibility to accommodate your schedule, with opportunities to set your own hours and commit to a minimum weekly or bi-weekly availability. Youll provide essential health evaluations to Medicare Advantage plan members throughout various neighborhoods within Broome County.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Set your own schedule based on availability
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments on Medicare Advantage members
- Collect comprehensive health histories, including current and prior medical conditions and medication usage
- Provide patient education regarding diagnoses, overall health status, and medication management
- Identify and report gaps in care, recommending appropriate next steps
- Focus exclusively on assessmentno direct creation of treatment plans or medication prescribing is necessary
- Average assessment time is approximately 45 minutes per visit
Qualifications:
- Current Nurse Practitioner license issued by the state of NY
- Active CPR certification (BLS)
- Board Certification as a Nurse Practitioner via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of recent experience as an NP (preferred)
In this role, youll earn between $80 and $90 for each evaluation, along with a mileage stipend and compensation for administrative time.
