This per diem role is ideal for Nurse Practitioners experienced in home health, looking for flexibility and professional autonomy while serving communities in Middleport, Pomeroy, Syracuse, Racine, and beyond.
Schedule:
- Flexible shifts available seven days a week
- Self-scheduling: create a schedule that works for you
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform home health risk assessments, each visit averaging 45 minutes
- Review medical history, chronic conditions, and medication regimens
- Educate patients on diagnoses and pharmacological therapies
- Identify gaps in care and make clear recommendations to enhance patient wellbeing
- Visits may include annual health assessments or post-discharge check-ins
- No treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- MSN from an accredited nursing school
- Current CPR (BLS) certification
- Minimum of six months NP experience preferred, particularly in home health or related settings
