Job Description

Step into a pivotal role as a Nurse Practitioner focused on home health risk assessments within Meigs County, Ohio. In this per diem position, you will have the flexibility to design your own work schedule, engaging in visits to Medicare Advantage plan members throughout Middleport, Pomeroy, Syracuse, and Racine.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Arrange your own schedule
  • Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes
  • Evaluate prior and current medical history, review existing medical conditions, and assess medication history
  • Deliver targeted education to members regarding medical conditions and prescribed medications
  • Identify gaps in care and provide actionable recommendations without the need for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited school of nursing
  • Minimum six months of current NP experience preferred

This opportunity offers $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
