This per diem position offers unmatched scheduling flexibilityyou choose your shifts between Monday and Sunday, allowing you to create a work-life balance that fits your lifestyle. Each assignment involves visiting Medicare Advantage plan members in their homes, performing detailed risk assessments averaging 45 minutes per visit. You may be scheduled at various sites within Cumberland County based on shift needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients medical history, current health conditions, and medication regimens
• Educate patients on disease prevention, condition management, and medication adherence
• Identify care gaps and propose actionable recommendations
• Focus exclusively on assessments; no diagnosis, prescribing, or treatment planning required

Qualifications:

• Valid Nurse Practitioner license in Maine
• MSN from an accredited nursing program
• Current certification in Basic Life Support (BLS/CPR)
• Board certified through AANP or ANCC
• At least six months of recent nurse practitioner experience preferred
