Job Description
Deliver compassionate care in the homes of patients as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, you'll engage directly with Medicare Advantage plan members across Broome County, NYincluding areas such as Binghamton, Vestal, and Johnson Cityto evaluate their health and ensure the best possible outcomes post-discharge or during annual check-ups.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to minimum weekly or bi-weekly hours as per your availability
Key Responsibilities:
- Perform comprehensive annual risk assessments in patient homes
- Review and document prior medical history, current health conditions, and medication regimens
- Offer guidance and education to patients regarding their health conditions and medications
- Identify gaps in care, making practical recommendations for follow-up
- Focus on assessmentno need to create treatment plans or prescribe medications
Qualifications:
- Licensed Nurse Practitioner in New York
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of 6 months of recent experience as a Nurse Practitioner preferred
- MSN from an accredited nursing school
