Step into a dynamic, per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this role, you'll engage directly with Medicare Advantage members throughout Duplin County, conducting comprehensive health evaluations during home visits. Key Responsibilities:

• Perform annual health risk assessments in patient homes for a Medicare Advantage plan
• Review prior and current medical history, assess medical conditions, and analyze medication usage
• Deliver patient education regarding health conditions and pharmacological regimens
• Identify gaps in care and recommend steps to address them
• Focus on evaluation onlyno prescribing or treatment planning necessary

Qualifications:

• Active and unrestricted Nurse Practitioner license in NC
• Current CPR certification (BLS)
• Board Certified through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of current NP experience preferred
