Job Description

Make a direct impact on patient health by performing home health risk assessments for Medicare Advantage members across New Haven County, CT. Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment to fit your availability

Key Responsibilities:

  • Conduct comprehensive home health risk assessments (approximately 45 minutes each), including annual wellness and post-discharge visits
  • Review and evaluate patients prior/current medical history, medical conditions, and medication regimens
  • Deliver clear patient education related to diagnoses and medication usage
  • Identify potential gaps in care and provide actionable recommendations
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner license in Connecticut
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited program
  • Minimum six months of recent Nurse Practitioner experience is preferred
