Make a direct impact on patient health by performing home health risk assessments for Medicare Advantage members across New Haven County, CT. This per diem opportunity empowers you to use your Nurse Practitioner expertise in a highly flexible, patient-focused settingwhere each visit strengthens preventative care and patient education.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment to fit your availability
Key Responsibilities:
- Conduct comprehensive home health risk assessments (approximately 45 minutes each), including annual wellness and post-discharge visits
- Review and evaluate patients prior/current medical history, medical conditions, and medication regimens
- Deliver clear patient education related to diagnoses and medication usage
- Identify potential gaps in care and provide actionable recommendations
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in Connecticut
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- Minimum six months of recent Nurse Practitioner experience is preferred
