Job Description

Seeking an experienced nurse practitioner to provide essential home health risk assessments for Medicare Advantage plan members throughout Carroll County, AR, including Berryville, Green Forest, and Holiday Island. This per diem opportunity allows you to make a meaningful impact by ensuring comprehensive care for patients where they live.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to self-schedule
  • Minimum weekly or bi-weekly commitment required

Key Responsibilities:

  • Complete annual home health risk assessments during patient visits (approximately 45 minutes each)
  • Review and document prior and current medical history, including conditions and medications
  • Educate members regarding their health conditions and prescribed treatments
  • Identify care gaps and suggest appropriate next steps to address them
  • Visits may consist of annual health assessments or post-discharge evaluationsno prescribing or treatment planning involved

Qualifications:

  • Active and unrestricted nurse practitioner license in Arkansas
  • Board certified by AANP or ANCC
  • Current BLS (CPR) certification
  • Master of Science in Nursing (MSN) from an accredited program
  • Minimum of six months NP experience preferred, with background in home health a plus
