Advance your Nurse Practitioner career by providing impactful care in a flexible, per diem role conducting home health risk assessments throughout Adams County, IN. This position empowers you to design your own work schedule, choosing shifts between Monday and Sunday, with the convenience of self-managed weekly or bi-weekly hour commitments. Travel to various patient homes in locations such as Decatur, Berne, and Geneva to perform comprehensive, one-on-one health assessments with Medicare Advantage plan members.
Responsibilities include:
- Conducting annual home health risk assessments for Medicare Advantage health plan members
- Evaluating medical history, current conditions, and medication profiles
- Educating patients on their conditions and prescribed medications
- Identifying gaps in care and offering clear recommendations to bridge them
- Performing thorough assessmentseach typically lasting about 45 minutes
- No requirement to develop treatment plans or prescribe medications
Ideal candidates will bring:
- Active Nurse Practitioner license in Indiana
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- At least 6 months of recent nurse practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited program
