Deliver impactful healthcare directly to the community through meaningful one-on-one patient interactions. This per diem opportunity offers Nurse Practitioners the chance to conduct comprehensive home health risk assessments for Medicare Advantage plan members throughout Benton County, including Bentonville, Rogers, and Bella Vista.
Schedule:
- Flexible schedulingshifts available seven days a week

- Minimum weekly or bi-weekly hourly commitments for added flexibility
Key Responsibilities:
- Perform thorough annual home health risk assessments in patient homes
- Review and evaluate patients prior and current medical history, diagnoses, and medication use
- Educate patients on their condition management and medications
- Identify gaps in care and recommend appropriate next steps to optimize patient health outcomes
- No treatment planning or prescribing responsibilities
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC required
- At least 6 months of experience as a practicing NP preferred
- MSN from an accredited nursing program
