Make a positive impact in patient care by delivering comprehensive home health risk assessments on a flexible, per diem schedule. In this role, youll visit Medicare Advantage plan members across Pasco County and the surrounding areas of Franklin County, including locations such as Connell, Kahlotus, and Mesa. Assignments vary by shift and may include annual or post-discharge assessments, each lasting about 45 minutes.

Schedule:

• Flexible shifts available Monday through Sunday
• Build your schedule around your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform thorough annual home health risk assessments for Medicare Advantage members
• Review and document detailed medical, surgical, and medication history
• Educate patients about their medical conditions and prescribed treatments
• Collaborate to identify care gaps and provide actionable recommendations
• Focus on assessment and educationno prescribing or treatment plan creation required

Qualifications:

• Active Nurse Practitioner license in Washington State
• Board Certification through AANP or ANCC
• Current BLS/CPR certification
• Minimum of six months recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
