Shifts are scheduled within Chesterfield County, covering areas such as Cheraw, Chesterfield, and Pageland.

Schedule:

• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments in the members home setting
• Evaluate both prior and current medical history, existing conditions, and medication usage
• Deliver clear education on health conditions and prescribed medications
• Identify any care gaps and provide actionable recommendations to address them
• No treatment plan creation or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in South Carolina
• Board certified through AANP or ANCC
• MSN degree from an accredited institution
• Current BLS (CPR) certification
• Preferred: At least 6 months of recent Nurse Practitioner experience

This role is per evaluation, offering $80-$90 for each assessment completed, along with a mileage stipend and compensation for administrative time.
