Job Description

As a skilled Nurse Practitioner with home health experience, youll play a vital role in advancing patient care throughout Winnebago County, Illinois. This per diem opportunity offers autonomy and flexibility, giving you the ability to design your own schedule and impact the health of Medicare Advantage plan members through in-home risk assessments.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose shifts that work for you
  • Minimum weekly or bi-weekly hourly commitment tailored to your availability

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members, addressing both new and post-discharge patients
  • Review and evaluate prior and current medical history, conditions, and medication use
  • Educate patients and their families about diagnoses, medications, and care strategies
  • Identify gaps in care and offer actionable recommendations for improvement
  • Visits may include a variety of locations within Winnebago County such as Rockford, Loves Park, or Machesney Park
  • No requirement to develop treatment plans or prescribe medications, focusing on assessment and education

Required Qualifications:

  • Active Nurse Practitioner licensure in Illinois
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • Minimum of 6 months experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program
