Advance your clinical expertise while making a meaningful impact in home-based healthcare within Washington County, TX. This per diem opportunity is ideal for a Nurse Practitioner seeking a flexible schedule and autonomy while delivering home health risk assessments to Medicare Advantage plan members in communities such as Brenham, Mount Pleasant, Winfield, Talco, and other areas within the county.
Schedule:
- Flexible shifts available Monday through Sunday
- Enjoy the ability to create your own work schedule
- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for a Medicare Advantage health plan
- Review and evaluate medical history, existing conditions, and medication usage during each home visit
- Deliver clear education to patients regarding their health conditions and medications
- Identify care gaps and make recommendations for improvement and follow-up
- All visits are assessment-basedno treatment planning or prescriptions required
Qualifications:
- Current Nurse Practitioner license in the state of Texas
- BLS certification (CPR) is required
- Board certification through AANP or ANCC
- A minimum of six months NP experience preferred
- MSN degree from an accredited nursing program
