Job Description
Experienced Nurse Practitioners are needed for a per diem home health position where you'll conduct professional risk assessments with flexibility and autonomy. This role involves traveling throughout Madison County, TNincluding Jackson, Medon, and Three Wayto visit Medicare Advantage plan members in their homes. Each evaluation averages 45 minutes and may be an annual assessment or a post-discharge check-up.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Set your own work schedule
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete annual risk assessments for Medicare Advantage members during home visits
- Review and evaluate prior and current medical histories, conditions, and medication use
- Educate members about medical conditions and medications
- Identify care gaps and recommend solutionsno treatment plans or prescriptions required
Qualifications:
- Nurse Practitioner license valid in TN
- Current CPR certification (BLS)
- Board Certification from AANP or ANCC
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Professionals in this role can expect compensation of $80$90 per evaluation, as well as a mileage stipend and paid administrative time.
