Advance your clinical expertise while maintaining control over your schedule as a Nurse Practitioner providing per diem home health risk assessments. This independent opportunity allows you to deliver vital care to Medicare Advantage plan members across multiple communities within King County. Assessments take place in the comfort of each members home and are scheduled throughout areas such as Redmond, Kirkland, Federal Way, and neighboring Seattle regions.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule around your other commitments
• Choose your preferred hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

• Complete home-based annual health risk assessments, evaluating overall medical and medication histories for Medicare Advantage plan members
• Educate patients on their health conditions and prescribed medications
• Identify and report any care gaps, offering recommendations to improve health outcomes
• Conduct visits that may range from annual health assessments to post-discharge evaluations
• No creation of ongoing treatment plans or prescription writing required

Qualifications:

• Active and unrestricted Nurse Practitioner license in Washington State
• Board certification (AANP or ANCC)
• Master of Science in Nursing (MSN) from an accredited institution
• Current CPR (BLS) certification
• At least six months of experience practicing as a nurse practitioner is preferred
