Make a difference in patient care by providing essential home health risk assessments for Medicare Advantage members. This per diem opportunity offers unmatched scheduling flexibility and the chance to deliver impactful care in various communities throughout Erie County, including Sandusky, Huron, and Vermilion. As a skilled Nurse Practitioner, you'll perform assessments in patients' homesaverage visit duration is approximately 45 minutes, with each visit tailored to annual wellness, post-discharge, or other health needs.

Schedule:

• Flexible shifts available seven days a week

• Opportunity to choose minimum weekly or bi-weekly commitment

Primary Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review prior and current medical history, existing conditions, and medication use
• Educate patients on managing medical conditions and appropriate medication usage
• Identify and address care gaps, offering recommendations to promote patient health
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR (BLS) certification
• Board certified through AANP or ANCC
• Six months or more of recent Nurse Practitioner experience preferred
• MSN from an accredited nursing program

For each evaluation, compensation ranges from $80 to $90, with additional mileage stipends and paid administrative time provided.
