Job Description

Seeking a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members throughout St. Clair County, Illinois. This rewarding per diem opportunity provides autonomy and work-life balance, as you design your own flexible schedule and gain experience working with a dynamic patient population across Belleville, OFallon, East St. Louis, and nearby areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to balance work and personal commitments
  • Commit to minimum weekly or bi-weekly hours based on your availability

Key Responsibilities:

  • Conduct annual home health risk assessments in patient homes
  • Gather and review prior/current medical histories, existing conditions, and medication records
  • Provide patient education on medical conditions, medications, and preventative health
  • Identify care gaps and recommend strategies for resolution
  • No chronic care management, treatment planning, or medication prescribing required

Qualifications:

  • Valid Nurse Practitioner license in Illinois
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • 6+ months of current Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program
