Seeking a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members throughout St. Clair County, Illinois. This rewarding per diem opportunity provides autonomy and work-life balance, as you design your own flexible schedule and gain experience working with a dynamic patient population across Belleville, OFallon, East St. Louis, and nearby areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to balance work and personal commitments
- Commit to minimum weekly or bi-weekly hours based on your availability
Key Responsibilities:
- Conduct annual home health risk assessments in patient homes
- Gather and review prior/current medical histories, existing conditions, and medication records
- Provide patient education on medical conditions, medications, and preventative health
- Identify care gaps and recommend strategies for resolution
- No chronic care management, treatment planning, or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in Illinois
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- 6+ months of current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
