Job Description

Play a critical role in improving health outcomes for Medicare Advantage plan members by conducting comprehensive home health risk assessments in Madison County, Illinois. This per diem opportunity allows Nurse Practitioners to leverage clinical expertise in the field providing flexible scheduling and autonomy with impactful work.

Schedule:

  • Flexible shifts available seven days a week
  • Self-created scheduling to fit your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members in various Madison County communities, including Granite City, Edwardsville, and Alton
  • Evaluate patient medical history, current conditions, and medication use during visits
  • Educate patients regarding their health conditions and medication regimens
  • Identify care gaps and recommend solutions to optimize patient health
  • Perform documentation; treatment planning or prescribing is not required
  • Each risk assessment typically lasts about 45 minutes, with visits including annual checks or post-discharge evaluations

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board certification through AANP or ANCC
  • Completion of an MSN degree from an accredited nursing program
  • Current BLS (CPR) certification
  • Minimum six months of recent Nurse Practitioner experience preferred

In this highly flexible, field-based position, dedicated Nurse Practitioners will receive $80$90 per evaluation, a mileage stipend, and compensation for administrative time.
