Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members in areas including Edwardsville, Alton, and Granite City
  • Evaluate patients prior and current medical history, ongoing conditions, and medication regimens during approximately 45-minute visits
  • Educate patients on health conditions, medication regimens, and preventive health strategies
  • Identify care gaps and make recommendations to ensure the best outcomes
  • Post-visit requirements do not include treatment planning or prescription writing

Qualifications:

  • Active Nurse Practitioner License in Illinois
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing school
  • Minimum of 6 months NP experience preferred
