Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members in areas including Edwardsville, Alton, and Granite City
- Evaluate patients prior and current medical history, ongoing conditions, and medication regimens during approximately 45-minute visits
- Educate patients on health conditions, medication regimens, and preventive health strategies
- Identify care gaps and make recommendations to ensure the best outcomes
- Post-visit requirements do not include treatment planning or prescription writing
Qualifications:
- Active Nurse Practitioner License in Illinois
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing school
- Minimum of 6 months NP experience preferred
