Job Description

Expert home health practitioners play a pivotal role in supporting Medicare Advantage members by delivering comprehensive risk assessments in the comfort of their homes. This per diem opportunity is designed for those with a passion for patient-centered care and flexibility in their scheduling.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to design your own schedule
  • Minimum commitment required, offered on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage plan members across Marion County, including Ocala, Belleview, and Dunnellon.
  • Review medical and medication histories, evaluate current medical conditions, and educate members about their health and prescribed treatments.
  • Identify care gaps and recommend strategies for improved health outcomes.
  • No responsibility for developing treatment plans or prescribing medicationsfocus is assessment and education.
  • Each home visit typically takes about 45 minutes, with assessments varying from annual reviews to post-discharge visits.

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current BLS CPR certification
  • Board Certification with AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience preferred

Compensation & Benefits: Practitioners are compensated $80-$90 per completed evaluation, in addition to mileage reimbursement and compensation for administrative time.
