Deliver high-quality patient care directly in the homes of Medicare Advantage plan members throughout Fayette County, PA, including areas such as Uniontown, Connellsville, and Masontown. This per diem nurse practitioner opportunity provides the autonomy to manage your own schedule while making a positive impact on the health and well-being of the local community.

Schedule:

• Flexible shifts available Monday through Sunday

Key Responsibilities:

• Conduct detailed annual home health risk assessments for Medicare Advantage members
• Assess prior and current medical histories, review medical conditions, and evaluate medication regimens
• Educate patients regarding their health conditions and prescribed medications
• Identify care gaps and suggest recommendations for improved patient outcomes
• Focus exclusively on assessments; no treatment plan development or medication prescribing required

Qualifications:

• Current Nurse Practitioner license in Pennsylvania
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• MSN degree from an accredited nursing program
• At least 6 months of recent experience as a Nurse Practitioner preferred
