Job Description
Make a meaningful impact by conducting home health risk assessments for Medicare Advantage plan members throughout Hendry County, FL. As a Nurse Practitioner in this per diem opportunity, your expertise supports individuals in Clewiston, LaBelle, Montura, and surrounding communities, assessing health needs in the comfort of patients own homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign your own workweek
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform comprehensive annual home health risk assessmentseach taking about 45 minutes
- Review and evaluate prior and current medical history, conditions, and medication lists
- Provide clear education on medical conditions and medications to patients
- Identify any gaps in care and offer recommendations to close them
- Focus solely on assessmentno requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification through AANP or ANCC required
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited nursing program
- At least six months of recent NP experience preferred
