Job Description

A dynamic opportunity is available for an experienced Nurse Practitioner to provide essential home health risk assessments in Benton County. This per diem position offers maximum flexibility, allowing you to set your own schedule with shifts available any day of the week. Youll be traveling to locations such as Rogers, Bentonville, and Bella Vista, conducting comprehensive risk evaluations for Medicare Advantage plan members in the comfort of their homes.

Schedule:

  • Set your preferred workdays, Monday through Sunday
  • Flexibility to create your own schedule
  • Minimum hourly commitment required each week or bi-weekly

Key Responsibilities:

  • Perform in-home annual health risk assessments and post-discharge follow-up visits, lasting approximately 45 minutes per patient
  • Review and evaluate prior and current medical conditions, medication regimens, and overall medical history
  • Identify gaps in care and offer evidence-based recommendations for closing those gaps
  • Provide patient education regarding their health conditions and medications
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Board Certification from AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current BLS/CPR certification
  • Minimum of 6 months current experience as a Nurse Practitioner is strongly preferred
