Exciting opportunities are available for highly skilled Nurse Practitioners to perform home health risk assessments. This per diem position offers the flexibility to craft your own schedule, with available shifts spanning Monday through Sunday and the ability to set your preferred weekly or bi-weekly time commitment.

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members throughout Calcasieu County, including areas such as Lake Charles, Sulphur, and DeQuincy
- Evaluate patients' prior and current medical history, assess ongoing medical conditions, and review medication usage
- Offer educational guidance on various health conditions and prescribed treatments
- Identify gaps in care and provide recommendations to enhance patient outcomes
- Focus on assessment and educationno requirements to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Louisiana
- MSN degree from an accredited school of nursing
- Board certification via AANP or ANCC
- Current BLS (CPR) certification
- At least 6 months of recent NP experience is preferred
