As a Nurse Practitioner specializing in home health, you'll play a vital role in ensuring comprehensive care for Medicare Advantage members across Calcasieu County, Louisiana, including Lake Charles, Sulphur, and DeQuincy. This per diem opportunity allows you to tailor your own schedule, completing home health risk assessments in patient residences. Each evaluation typically lasts around 45 minutes, focusing on annual health reviews or post-discharge visits.
Schedule:
- Flexible shifts available from Monday through Sunday
- Create your own schedule
- Choose your minimum weekly or bi-weekly commitment
Key Responsibilities:
- Conduct thorough home health risk assessments for Medicare Advantage members
- Review prior and current medical histories, evaluate medical conditions, and carefully assess medication histories
- Provide clear and concise education to patients regarding their health conditions and medications
- Highlight care gaps and advise on steps for resolution
- No requirement for developing treatment plans or prescribing medications
Desired Qualifications:
- Current, unrestricted Nurse Practitioner license in Louisiana
- Active CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of six months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
