Advance your career by providing essential home health services as a Nurse Practitioner specializing in home-based risk assessments. This per diem position offers the opportunity to support Medicare Advantage members across Mason County, WAincluding areas such as Shelton, Arcadia, Kamilche, and Matlockwhile enjoying maximum flexibility in your scheduling.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to design your own schedule
- Minimum hourly commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Evaluate patient histories, including prior and current medical conditions as well as medication use
- Educate patients regarding chronic or acute medical issues and appropriate medication management
- Identify gaps in care and provide actionable recommendations to enhance care delivery
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Washington
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months of NP experience preferred
- MSN degree from an accredited nursing program
In this role, you will be compensated $80-$90 per completed evaluation, along with a mileage stipend and payment for administrative time associated with your visits.
