Youll focus on home health risk assessments for Medicare Advantage members across Harford County, serving residents in locations such as Aberdeen, Bel Air, and Edgewood.

Schedule/Hours:

• Choose your own shifts with complete flexibility between Monday and Sunday
• Set your own schedule to suit your lifestyle
• Minimum weekly or bi-weekly hourly commitments available

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review and document detailed medical, medication, and health histories
• Provide education on diagnoses, treatment options, and medications
• Identify care gaps and recommend solutions to promote optimal health
• Visits may include annual assessments, as well as post-discharge evaluations
• No treatment plan development or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Maryland
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR (BLS) certification
• Minimum of 6 months NP experience preferred
