Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your needs
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage members in various communities throughout the county
- Comprehensive evaluation of medical history, current conditions, and medication use
- Educate patients on their medical conditions, prescribed therapies, and preventive health strategies during the visit
- Identify care gaps and provide recommendations to help address these areas
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license (TN)
- Board Certification through AANP or ANCC
- Masters of Science in Nursing (MSN) from an accredited institution
- Current CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience is preferred
Enjoy compensation of $80$90 for each evaluation completed, along with a mileage stipend and paid administrative time.
