Job Description

Enjoy the flexibility and rewards of providing care in a per diem home health role, focused on risk assessments for Medicare Advantage plan members. As a Nurse Practitioner, you will bring expertise and compassion directly to patients in the comfort of their own homes, helping ensure the best possible outcomes for the communities of Henderson County.

Schedule:

  • Flexible shifts available seven days a week
  • Design your own work schedule to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct comprehensive home health risk assessments, each taking approximately 45 minutes
  • Travel to patient residences in locations such as Lexington, Parkers Crossroads, and Sardis within Henderson County
  • Review and document prior and current medical history, existing conditions, and medication use
  • Educate patients on health conditions and proper medication usage during visits
  • Assess for gaps in care, provide professional recommendations, and support overall health plan goals
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner license in Tennessee
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • Minimum of six months experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited program
