Advance your career as a Nurse Practitioner in a dynamic per diem opportunity focused on home health risk assessments. Serve Medicare Advantage members by conducting in-home visits throughout the Multnomah County area, including Portland, Gresham, Troutdale, and Fairview. With a flexible scheduling model and the ability to create your own shifts, this role offers autonomy and meaningful patient interaction.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your preferred schedule week to week
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan enrollees
• Review and evaluate patients prior and current medical histories, medical conditions, and medication usage
• Educate patients about their health conditions and medications to empower self-care
• Recognize gaps in care and offer actionable recommendations, enhancing care quality
• No requirement to create treatment plans or prescribe medicationsfocus on evaluation and education

Qualifications:

• Active Nurse Practitioner license in Oregon
• Current BLS/CPR certification
• Board certification from AANP or ANCC
• MSN from an accredited nursing school
• At least six months experience as a Nurse Practitioner preferred

Additional Incentives:

• Earn between $80-$90 per evaluation, with an additional mileage stipend and compensation for administrative time
