Providing comprehensive care for seniors in home and senior living settings, this Nurse Practitioner role focuses on treating patients in place to reduce avoidable hospitalizations and emergency room visits. With an emphasis on monthly health assessmentsmore frequent as neededthis position ensures that care plans and resources effectively support patients health in a comfortable and familiar environment.
Schedule:
- Monday to Friday, 8 AM 5 PM
- No weekends or holidays

Key Responsibilities:
- Assess patient health regularly, creating and updating problem lists and individualized medical treatment plans
- Provide primary, preventive, urgent, and acute care on-site within senior living communities and private homes
- Identify and stratify high-risk patients to increase clinical visits and closely monitor their conditions
- Develop contingency plans to swiftly address high-risk health issues
- Collaborate effectively with patients primary care physicians, facility staff, and Directors of Nursing (DON)
- Deliver bedside care, formulate care plans, and initiate orders to improve patient outcomes
- Conduct single visits for patients discharged from skilled nursing facilities as part of their transition care
Qualifications:
- Master of Science in Nursing (MSN) degree
- Current Oregon Nurse Practitioner license
- DEA certification
- Basic Life Support (BLS) certification
- Preferred experience in Home Health or Hospice settings
