Make a meaningful impact in patient care by providing comprehensive home health risk assessments for Medicare Advantage plan members. This per diem opportunity prioritizes autonomy and flexibility, allowing you to set your own weekly or bi-weekly schedule with shifts available any day between Monday and Sunday.

Schedule:

• Flexible shifts across all days of the week

Key Responsibilities:

• Perform in-home health assessmentseach typically lasting about 45 minutesfor older adults residing in various communities across Fayette County, including Oak Hill, Fayetteville, and Montgomery
• Review medical history, active conditions, and medication usage during annual assessments or post-discharge visits
• Provide clear, actionable education regarding diagnoses and medications to patients and caregivers
• Detect gaps in healthcare, offering recommendations to bridge those gaps; no medication prescribing or treatment planning required

Qualifications:

• Current Nurse Practitioner license in West Virginia (WV)
• Certified in Basic Life Support (CPR/BLS)
• Active Board Certification via AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience is strongly preferred
