This per diem opportunity offers remarkable flexibility and autonomycreate your own schedule with shifts available between Monday and Sunday, and choose from a minimum weekly or bi-weekly hourly commitment.
Responsibilities include conducting comprehensive annual health risk evaluations for Medicare Advantage plan members within their homes, spanning locations such as Morehead City, Beaufort, and Newport. Each appointment lasts approximately 45 minutes and may be an annual health evaluation or a post-discharge assessment. Your focus will be to:
- Complete medical and medication history reviews
- Assess and document both prior and current health conditions
- Educate patients on their conditions and medications, promoting better health outcomes
- Identify care gaps and recommend next steps as needed
- Note: No treatment planning or prescription activities are required
Candidates should meet these qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board Certification from AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of nurse practitioner experience preferred
