Advance your career in home health by conducting in-depth assessments as a Nurse Practitioner with a flexible per diem schedule. This role involves visiting Medicare Advantage plan members for comprehensive home health risk evaluations in Dallas County, including Fordyce, Sparkman, and Carthage. Each initial or follow-up assessment typically takes about 45 minutes and may include annual health reviews or post-discharge visits. Enjoy the autonomy to make your own schedule and build meaningful connections with patients in their homes.
Schedule:
- Flexible shifts available MondaySunday
- Create your own schedule
- Weekly or bi-weekly hourly commitments, based on your availability
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and document patients medical history, existing conditions, and medication regimens
- Deliver clear education to members regarding their conditions and prescribed medications
- Identify gaps in care and recommend appropriate next steps
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license (AR)
- Board certification (AANP or ANCC)
- MSN from an accredited program
- Valid BLS certification (CPR)
- At least 6 months of NP experience (preferred)
