Job Description

Deliver comprehensive patient care in a flexible, community-based role as a per diem Nurse Practitioner, conducting home health risk assessments across Ouachita County, ARincluding Camden, Stephens, and Bearden. This position offers the opportunity to work directly with Medicare Advantage plan members, focusing on both annual evaluations and post-discharge assessments. Home visits typically last about 45 minutes, ensuring efficient yet thorough patient interactions.

  • Flexible shifts available Monday through Sunday

  • Commit to a minimum weekly or bi-weekly hourly requirement

Key Responsibilities:

  • Perform home health risk assessments for Medicare Advantage members
  • Assess prior and current medical histories, chronic conditions, and medications
  • Educate patients about diagnoses, treatments, and medication management
  • Identify gaps in care; offer actionable recommendations to address these gaps
  • Note: This role does not involve developing treatment plans or prescribing medications

Qualifications:

  • Current Nurse Practitioner license in Arkansas
  • Active CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Minimum of six months experience as a Nurse Practitioner preferred
  • MSN from an accredited nursing program
