Bring your advanced clinical skills to home health care as a Nurse Practitioner providing risk assessments for Medicare Advantage members throughout Spotsylvania County. Youll visit patients in their homeslocations may include Spotsylvania Courthouse, Lake Wilderness, and other local areasto conduct comprehensive assessments that help ensure the highest level of care.

Schedule:

• Flexible shifts available Monday through Sunday

• Opportunities to commit minimum weekly or bi-weekly hours

Key Responsibilities:

• Perform home-based risk assessments, generally completed within a 45-minute timeframe
• Review and evaluate comprehensive medical histories, ongoing conditions, and medication regimens
• Deliver education to patients on their medical conditions and prescribed medications
• Identify gaps in care and recommend actions to address these needs
• Focus on assessments without developing treatment plans or prescribing medications

Qualifications:

• Current Nurse Practitioner license in Virginia
• Active CPR certification (BLS)
• Board certification through AANP or ANCC
• MSN from an accredited nursing school
• At least 6 months experience as a Nurse Practitioner preferred
