Position Overview: You will conduct home health risk assessments for Medicare Advantage plan members throughout Monmouth County, including areas such as Middletown, Howell, and Marlboro. Each evaluation typically lasts around 45 minutes and may address annual health assessments or post-discharge visits. The role involves traveling to patient homes to assess health history, review current conditions and medications, and educate patients about their careall without the need to create treatment plans or prescribe medications.
Responsibilities:
- Perform thorough annual risk assessments in patients homes
- Evaluate and document past and current medical history, conditions, and medications
- Provide clear education to patients on managing their health
- Identify gaps in care and offer recommendations for improvement
- Ensure accurate documentation after each visit
Preferred Qualifications:
- Active Nurse Practitioner License in New Jersey
- CPR certification (BLS required)
- Board Certification via AANP or ANCC
- MSN degree from an accredited nursing program
- A minimum of 6 months recent experience as a Nurse Practitioner is preferred
