Deliver impactful care to Medicare Advantage plan members as a Nurse Practitioner specializing in home health risk assessments throughout Craven County, NCincluding Havelock, Vanceboro, Bridgeton, New Bern, and surrounding areas. This per diem opportunity allows you to make a meaningful difference by traveling to members' homes and performing focused, one-on-one assessments, all while enjoying the flexibility to design your own work schedule.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct in-home annual risk assessments for Medicare Advantage health plan members
• Review and evaluate prior and current medical history, chronic conditions, and medication use
• Provide important education to patients on their health conditions and medication regimen
• Identify gaps in care and recommend solutions to promote better patient outcomes
• Visits may include annual health assessments or post-discharge evaluations
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification through AANP or ANCC
• MSN degree from an accredited school of nursing
• Current CPR/BLS certification
• Minimum six months NP experience preferred

Compensation for this role is $80$90 per evaluation, plus a mileage stipend and administrative time.
