This per diem opportunity is ideal for an experienced Nurse Practitioner interested in conducting home health risk assessments throughout locations in Craven County, NC, such as New Bern, Havelock, Vanceboro, and Bridgeton. Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own working days and hours
- Meet a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual risk assessments in the home setting for Medicare Advantage plan members
- Review and evaluate prior and current medical histories, medications, and health conditions
- Educate members on their health status and prescribed medications
- Identify gaps in care and communicate actionable recommendations for improvement
- Visits may involve either annual health assessments or post-discharge evaluations
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification (AANP or ANCC)
- Current BLS/CPR certification
- MSN from an accredited school of nursing
- At least six months of recent NP experience is preferred
Compensation for this role is $80$90 per evaluation, which also includes a mileage stipend and additional pay for administrative time.
