Seeking a skilled Nurse Practitioner to perform home health risk assessments on Medicare Advantage plan members throughout Grant County. This per diem opportunity provides outstanding flexibility and the chance to make a meaningful impact in patient care, with visits to areas such as Bayard-Santa Rita, Santa Clara, Tyrone, Hurley, and Roswell.

Schedule:

• Flexible shifts available, Monday through Sunday
• Self-selected schedulingchoose the days and times you work
• Minimum hourly commitment on a weekly or bi-weekly basis

Responsibilities:

• Complete home health risk assessments, including annual wellness and post-discharge visits
• Review and evaluate patients medical and medication histories
• Educate patients on diagnosed conditions and prescribed medications
• Identify gaps in care and make recommendations to address them
• Documentation and reporting as requiredno treatment planning or prescription writing necessary

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• MSN from an accredited nursing school
• At least 6 months of current experience as a Nurse Practitioner (preferred)

Enjoy highly competitive, evaluation-based compensation$80-$90 for each risk assessment completed, plus mileage reimbursement and compensation for administrative time.
