Schedule:

- Commitment options include minimum weekly or bi-weekly hours
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior and current medical history, conditions, and all medications
- Deliver clear, concise education to patients regarding diagnoses and prescribed medications
- Identify gaps in patient care and provide recommendations to bridge these gaps
- Conduct post-discharge visits as needed, ensuring patient stability and continuity of care
- This role does not require treatment planning or prescription-writing responsibilities
Qualifications:
- Active Nurse Practitioner license in the state of Tennessee
- CPR certification (BLS) required
- Board Certification through AANP or ANCC
- Minimum of six months nurse practitioner experience preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
