Job Description
Deliver high-quality patient care while enjoying flexible scheduling as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will play a crucial role in improving health outcomes for Medicare Advantage plan members throughout Putnam County, TN, including areas such as Cookeville, Algood, and Monterey.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members in their residences
- Review patients previous and current medical history, ongoing medical conditions, and medication regimens
- Educate patients on their medical conditions and prescribed medications, empowering them to manage their health effectively
- Identify care gaps and make actionable recommendations to help resolve them
- No treatment plan development or medication prescribing required; focus remains on assessments and education
Qualifications:
- Current Nurse Practitioner license in the state of Tennessee
- Up-to-date CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months of recent NP experience preferred
- MSN degree attained from an accredited nursing program
In this role, you will receive $80-$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time. This position offers both autonomy and fulfillment, allowing you to set your own pace while making a meaningful difference within the community.
