A skilled Nurse Practitioner is needed to conduct home health risk assessments for Medicare Advantage plan members throughout Edgecombe County, NC. This per diem position involves travel within the community, performing evaluations in areas such as Tarboro, Lower Town Creek, and Princeville. Each assessment typically takes around 45 minutes and may include annual wellness visits or post-discharge evaluations, focusing on improving care for patients at home.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Option for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Complete home health risk assessments, including annual and post-discharge visits
• Assess prior and current medical history, existing conditions, and medication usage
• Educate patients about their conditions and prescribed medications
• Identify gaps in patient care and offer actionable recommendations
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing school
• At least 6 months of recent Nurse Practitioner experience preferred
