Job Description

Step into an impactful role conducting home health risk assessments as a Nurse Practitioner on a per diem basis. This position offers the opportunity to serve Medicare Advantage plan members throughout White County, Indiana, including communities such as Monticello, Brookston, and Monon. You will play a crucial part in identifying health risks and promoting quality care without the demands of developing treatment plans or prescribing medications.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Freedom to create your own schedule
  • Choose minimum weekly or bi-weekly hourly commitments

Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior/current medical history, existing medical conditions, and medication histories
  • Deliver education to patients regarding their health conditions and medications
  • Identify care gaps and communicate recommendations for improvement
  • No treatment planning or prescribing required

Qualifications:

  • Current Nurse Practitioner license in Indiana
  • Certified with either AANP or ANCC
  • Basic Life Support (BLS) CPR certification up to date
  • Graduation from an accredited MSN program
  • At least six months of recent Nurse Practitioner experience preferred
