This per diem opportunity is ideal for experienced Nurse Practitioners interested in home health risk assessments across White County, including areas such as Monticello, Brookston, and Monon.

Schedule/Hours:

• Flexible shifts available Monday through Sunday
• Self-select your workdays to fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients medical histories, current conditions, and medication use
• Educate individuals regarding their medical conditions and medication regimens
• Identify gaps in care and recommend actionable next steps for improved health outcomes
• Visits typically last 45 minutes and may include annual health assessments or post-discharge follow-ups
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board certification (AANP or ANCC)
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least six months of experience as a Nurse Practitioner preferred
