Take your nurse practitioner expertise into the community and make a meaningful impact with flexible, per diem home health opportunities. This dynamic role focuses on conducting home health risk assessments with Medicare Advantage plan members in Kalamazoo County, including Kalamazoo, Portage, and Oshtemo. Youll be able to set your own schedule, enjoying flexible shifts between Monday and Sunday, with minimum weekly or bi-weekly hour commitments.

Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients medical history, current conditions, and medication profiles
- Provide patient education on medical conditions and medication management
- Identify care gaps and make recommendations for improved care outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Michigan
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR certification (BLS)
- At least 6 months of recent nurse practitioner experience preferred
Youll conduct assessments at patients residences, each lasting about 45 minutes. Compensation is highly competitive: earn between $80$90 per evaluation, plus receive a mileage stipend and payment for administrative time.
