Job Description

An exciting opportunity is available for Nurse Practitioners to deliver high-quality care through home health risk assessments in Kalamazoo County, MI, including Kalamazoo, Portage, and Oshtemo. This flexible, per diem role is ideal for advanced practice clinicians seeking autonomy, variety, and the ability to make a meaningful difference for Medicare Advantage plan members.

Schedule:

  • Flexible shifts offered between Monday and Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitments, based on your availability

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members in their homes
  • Review and assess prior and current medical history, ongoing medical conditions, and medication usage
  • Provide patient education focused on health conditions, wellness, and safe medication practices
  • Identify gaps in care and recommend strategies for care improvementno treatment planning or medication prescribing required
  • Each evaluation averages 45 minutes and may include annual or post-discharge assessments

Qualifications:

  • Active Nurse Practitioner license in the state of Michigan
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing degree from an accredited program
  • Current BLS/CPR certification
  • At least 6 months of recent Nurse Practitioner experience preferred
  • Compassionate, independent, and able to communicate effectively with patients in their home settings

Enjoy compensation of $80-$90 per evaluation, along with a mileage stipend and administrative pay for your time outside of visits.
