Delivering essential healthcare to patients in the comfort of their own homes, this per diem role empowers Nurse Practitioners to conduct pivotal risk assessments for Medicare Advantage members throughout Kalamazoo County. Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling with minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members in locations such as Kalamazoo, Portage, and Oshtemo
  • Assess prior and current medical history, medical conditions, and medication history
  • Educate patients on their health conditions and prescribed medications
  • Identify any gaps in care, providing recommendations to enhance patient outcomes
  • Focus exclusively on evaluationsno treatment planning or prescribing required

Qualifications:

  • Valid Nurse Practitioner license in the state of Michigan
  • Active CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent experience as a Nurse Practitioner preferred
