Job Description
This per diem opportunity is designed for Nurse Practitioners seeking flexible scheduling and the chance to make a difference through home health visits. The position involves performing risk assessments for Medicare Advantage plan members in the Bath, NY area and throughout Steuben County, including locations such as Corning, Gang Mills, and Hornell.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to suit your needs
- Minimum commitment required weekly or bi-weekly
Key Responsibilities:
- Conduct annual home health risk assessments, typically lasting about 45 minutes per visit
- Evaluate patients prior and current medical and medication histories
- Provide education regarding medical conditions and prescribed medications
- Identify and communicate any gaps in care, suggesting resolutions as needed
- Visits may include annual health assessments and post-discharge follow-ups
- Note: This position does not require creating treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in NY
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of current NP experience is preferred
Compensation for this role is $80$90 per evaluation, with additional mileage stipend and compensation for administrative time.
