Job Description

Delivering valuable healthcare directly to patients' homes, this per diem nurse practitioner opportunity centers around conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Steuben County. Home visits may be scheduled in Bath, Corning, Gang Mills, Hornell, or surrounding communities. Each assessment typically lasts about 45 minutes, with some visits focusing on annual reviews and others on post-discharge follow-up.

Schedule:

  • Flexible shifts available Monday through Sunday
  • The ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage health plan members
  • Review patients prior and current medical histories, chronic conditions, and medication usage
  • Offer education and insights about individual medical conditions and prescriptions
  • Identify gaps in care and recommend practical solutions for improved health outcomes
  • No treatment planning or prescribing of medications is required

Qualifications:

  • Valid Nurse Practitioner (NP) license in New York
  • Board Certification through AANP or ANCC
  • MSN from an accredited school of nursing
  • Current CPR certification (BLS)
  • At least six months of recent nurse practitioner experience preferred

Compensation for this position includes $80$90 per completed evaluation, complemented by a mileage stipend and coverage for administrative time.
